Selective endothelin receptor antagonist (ERA) - selective ETA-antagonist

Description

First selective ETA-antagonist for chronic kidney disease (CKD) with proteinuria. Reduces blood pressure and proteinuria by blocking endothelin-mediated vasoconstriction and glomerular injury.

Mechanism of action

Selective blockade of ETA endothelin receptors on vascular smooth muscle and glomerular mesangial cells, reducing vasoconstriction, proteinuria, and renal inflammation. Avoids ETB receptor blockade to preserve endothelial function.
